Documents Requested

At the time of registration, parents and guardians must submit items 1-8:

  1. Parent/guardian photo identification
  2. Proof of Age – birth certificate, passport, etc.
  3. Immunization Records
    • Click here for immunizations required for K-12 registration.
    • Click here for immunizations required for preschool registration.
  4. Proof of Residency – must provide one proof from List 1 and one from List 2
  5. IEP, 504 and Evaluation – if student requires special education services
  6. Unofficial Transcript – high school transfer students only
  7. Parenting Plan/Custody Agreement/Guardianship, if applicable
  8. Emergency Contact Information

Proof of Residency

The parent or guardian must provide two (2) proofs of residency. One from List 1 and one from List 2. These documents should contain the parent/guardian's name and address. Electronic documents are accepted if they are current and contain the information required for the district to verify proof of residence (date, name, address, applicable signatures, etc.).

List 1 Documents

  • Deed or Mortgage Statement dated within 30 days of the date of submission
  • Real Estate Tax Receipt – dated within 1 year of the date of submission
  • Residential Lease/Rental Agreement – signed by landlord and tenant including the address, apartment number and date of rental
  • Sales Contract – free of contingencies, signed by all parties with a copy of the closing statement provided within 60 days of enrollment

List 2 Documents

  • Utility Bill – full statement dated within 30 days. Trash bill from most recent cycle
    • Accepted Utilities – water, gas, electric, sewer and trash
  • Verification of Service from a Utility Provider – 1st month bill required within 30 days
  • Personal Property Tax Receipt – dated within 1 year of the date of submission
  • Unexpired Occupancy Permit – listing all the occupants of the residence
  • Address Verification Paper – listing additional occupants
  • Government Acknowledgement of Address – dated within 30 days of the date of submission (i.e., social security, voter registration, etc.)
